ANS: YOUR 90 DAYS VALIDITY PERIOD IS YOUR MEMBERSHIP VALIDITY PERIOD WHICH IS RENEWABLE AFTER THE FIRST 60 DAYS, YOUR MEMBERSHIP CATEGORY DETERMINES THE WORTH OF YOUR ACCRUED WORTH OF BILL FOR THIS 90 DAYS VALIDITY PERIOD. IN AN EVENT WHERE YOU HAVE USED UP YOUR ACCRUED WORTH OF BILL WITHIN THE FIRST 30 DAYS OF YOUR VALIDITY PERIOD, YOU WILL BE EXPECTED TO RE-VALIDATE IMMEDIATELY TO INCREASE YOUR ACCRUED WORTH OF BILL TO ENABLE YOU TAKE FURTHER TREATMENT.
2. CAN I GO FOR TREATMENT OF ANY KIND, OR AN OPERATION FOR EITHER A MAJOR OR A MINOR CASE WITH ANY OF THE MEMBERSHIP CATEGORY ID CARD?
ANS: YES, ALL YOU NEED TO DO IS BUY THE N3,000 RECHARGE/RE-VALIDATION SVC SCRATCH CARDS AS MANY AS POSSIBLE THAT WILL RAISE YOUR ACCRUED WORTH OF BILL TO AN AMOUNT THAT SHOULD BE ABLE TO PAY FOR THE TREATMENT YOU ARE ABOUT TO CARRY OUT.
3. WHAT HAPPENS IF I DID NOT USE MY MEMBERSHIP CARD FOR TREATMENT WITHIN THE FIRST VALIDITY PERIOD OF 90 DAYS?
ANS: YOUR ACCRUED WORTH OF BILL WILL EXPIRE AFTER 90 DAYS VALIDITY PERIOD IF YOU DID NOT RE-VALIDATE YOUR MEMBERSHIP AFTER THE FIRST 60 DAYS. BUT IF YOU KEEP RE-VALIDATING YOUR MEMBERSHIP, YOUR ACCRUED WORTH OF BILL WILL KEEP ACCUMULATING UNTIL YOU MAKE USE OF IT ANYTIME OR ANY DAY YOU ARE IN NEED OF IT.
4. IF I RE-VALIDATE AFTER THE FIRST 60 DAYS, EVEN WHEN I HAVE NOT USE UP MY ACCRUED WORTH OF BILL, DOES IT STILL REMAINS VALID?
ANS: YES, AS LONG AS YOU KEEP RE-VALIDATING, YOUR ACCRUED WORTH OF BILL KEEPS INCREASING.
5. CAN MY ACCRUED WORTH OF BILLS BE PAID TO ME EVENTUALLY AS CLAIMS?
ANS: NO, YOU CAN ONLY USE IT TO SAVE YOUR LIFE AND THE LIVES OF ACCIDENT VICTIMS AS WELL AS BENEFICIARIES OF THE CARD MEMBERSHIP CATEGORY YOU CARRY.
6. HOW DO I KNOW IF MY ACCRUED WORTH OF BILL IS BEEN EXHAUSTED?
ANS: THERE ARE SEVERAL WAYS YOU CAN DO THIS,

7. IF I LOOSE MY MEMBERSHIP ID CARD, WHAT DO I DO?
ANS: YOU ARE ADVICE TO REPORT EVERY MEMBERSHIP ID CARD LOSS IMMEDIATELY, AND THEN MAKE FRANK EFFORT TO REPLACE IT IMMEDIATELY TO ENABLE YOU CONTINUE ACCESSING YOUR 100% HEALTH CARE.
8. IF A MEMBER DIES, CAN THE BENEFICIARIES CONTINUE TO USE THE MEMBERSHIP CARD OF THE DECEASED PERSON FOR TREATMENT?
ANS: NO, THE MEMBERSHIP CARD IS NOT TRANSFERABLE, WHAT HAPPENS IS THAT A MAXIMUM OF 4 OF THEIR CHILDREN, IF ANY WILL AUTOMATICALLY BENEFIT FROM THE SCHOLARSHIP PROGRAM OF THE SOCAFUND TRUST EDUCATIONAL FUND.
9. CAN I USE MY MEMBERSHIP ID CARD TO TREAT MYSELF OR AN ACCIDENT VICTIM AFTER IT HAS EXCEEDED ITS VALIDITY PERIOD OF 90 DAYS?
ANS: NO, IF YOU DID NOT RE-VALIDATE AFTER THE FIRST 60 DAYS, AT THE EXPIRATION OF THE 90 DAYS YOUR MEMBERSHIP WILL BE WITHDRAWN AND YOU CAN NO LONGER USE THE MEMBERSHIP ID CARD FOR ANY TREATMENT OR FOR ANY OTHER BENEFIT. YOUR CARD WILL BE ON THE RED LIGHT STATUS WHICH MEANS THAT THE CARD IS NO LONGER VALID FOR THE SVC 100% HEALTH CARE PROGRAM. IT WILL NO LONGER EXIT ON THE DATA BASE.
10. CAN A MEMBER'S BENEFICIARIES STILL BE ENTITLED TO ITS ACCRUED BENEFITS IN THE EVENT OF DEATH OF A MEMBER, IF THE MEMBER'S MEMBERSHIP EXPIRED BEFORE DEATH OCCURRED?
ANS: NO, ALL BENEFICIARIES OF A MEMBER WHO FAILS TO KEEP RE-VALIDATING WILL LOOSE ALL ACCRUED BENEFITS IN THE EVENT OF DEATH OF THAT MEMBER AS ONLY VALID MEMBERSHIP ID CARDS WILL BE CONSIDERED FOR SUCH BENEFITS.
11. IF A VALID MEMBER IS FACED WITH AN EMERGENCY, AND IT'S ACCRUED WORTH OF BILL IS BEEN USED UP OR NOT ENOUGH TO PAY THE BILLS, WILL HE/SHE STILL RECEIVE TREATMENT AT THIS INSTANCE?
ANS: YES, THIS WILL BE POSSIBLE ONLY IF IT'S AN EMERGENCY, AND THE BILL WILL BE PAID BY THE FOUNDATION, BUT THE MEMBER IS EXPECTED TO RE-VALIDATE IMMEDIATELY TO INCREASE IT'S ACCRUED WORTH OF BILL TO CLEAR UP THE BILLS AND ALSO PUT THEIR MEMBERSHIP CARD ON GREEN LIGHT STATUS. FAILURE TO RE-VALIDATE AFTER A 30 DAYS NOTICE WILL RESULT TO PUTTING THE MEMBER'S ID CARD ON THE RED LIGHT STATUS, WHICH MEANS THAT SUCH A MEMBER WILL BE DENIED ACCESS TO FURTHER TREATMENT UNDER THE SVC 100% HEALTH CARE PROGRAM.
12. HOW OFTEN CAN A MEMBER USE ITS MEMBERSHIP ID CARD FOR TREATMENT?
ANS: AS OFTEN AS POSSIBLE, AND AS LONG AS THEIR MEMBERSHIP ID CARD IS ON THE GREEN LIGHT STATUS.
13. CAN A MEMBER MIGRATE FROM ONE MEMBERSHIP CATEGORY TO ANOTHER?
ANS: YES, BUT ONLY TO A HIGHER CATEGORY.
14. IF THE HOSPITAL I CHOSE IS NOT YET ACCREDITED, WHAT DO I DO?
ANS: YOU CAN TEMPORALLY BE RECEIVING YOUR MEDICAL SERVICES FROM THE ONES ALREADY ACCREDITED IN YOUR STATE OF RESIDENCE UNTIL YOUR CHOICE IS EVENTUALLY ACCREDITED.
NOTE:
TO FULLY ENJOY THE SVC HEALTH CARE 100% DELIVERY, THE POLICY MUST BE STRICTLY FOLLOWED AND APPLIED.
ACCREDITED HOSPITALS NATION-WIDE WILL PROVIDE THE SERVICES OF THE SVC HEALTH CARE 100% DELIVERY TO ALL ITS MEMBERS FOLLOWING STRICTLY THE POLICY OF THE PROGRAM.
TO HAVE YOUR MEMBERSHIP ID CARD ON THE GREEN LIGHT STATUS, IT SIMPLY MEANS THAT YOU HAVE AN ACCRUED WORTH OF BILL THAT CAN GIVE YOU FREE ACCESS TO MEDICAL TREATMENT ANYTIME, ANY DAY.
TO HAVE YOUR MEMBERSHIP ID CARD ON THE RED LIGHT STATUS, IT SIMPLY MEANS THAT YOU CAN NO LONGER PRESENT SUCH AN ID CARD FOR ANY FORM OF TREATMENT OR BENEFITS WHAT SOEVER AS SUCH AN ID CARD WOULD HAVE BEEN DELETED FROM THE SVC DATA BASE. AND IN A SITUATION WHERE A MEMBER LOSES ITS MEMBERSHIP TWICE AS A RESULT OF NOT RE-VALIDATING HIS/HER MEMBERSHIP AS AT WHEN DUE, SUCH A PERSON CAN NO LONGER BE ACCEPTED AS A MEMBER OF THE SVC PROGRAM.
